Home and Community Care (HACC) Aged and Disability Support Funded by the NSW Department of Ageing, Disability and Home Care, Council auspices Home and Community Care (HACC) development workers to support aged and disability service providers in delivering services to meet the needs of older people, people with disabilities and their carers across the Riverina and Murray area. The Co-ordinator will: - Support age and disability community forums for aged and disability services across the region in order to provide the best possible services through co-ordination, communication and common assessment and referral procedures.
- Train and support service providers in the HACC (Home and Community Care) agenda, which includes new systems of assessment, referral, data collection and compliance to service standards.
- Respond to issues relating to older people, people with disabilities and their carers that come through Council.
